Urgent angioembolization with early elective nephron-sparing surgery for spontaneously ruptured renal angiomyolipoma.

چکیده
Tumor rupture with hemorrhage is a severe complication of renal angiomyolipoma. Urgent transcatheter arterial embolization is an effective method for the patient suffering from life-threatening hemorrhage due to ruptured angiomyolipoma. Elective surgery has been suggested because of the possibility of rebleeding or abscess formation after embolization. However, it is not easy to perform a nephron-sparing procedure by delayed elective surgery due to severe tissue adhesion after bleeding. Herein, we report 2 cases of ruptured renal angiomyolipoma initially managed with transcatheter arterial embolization. Nephron-sparing surgery was performed a few days after embolization, before tissue adhesion occurred. Both kidneys were preserved, and the postoperative course was uneventful.

Laparoscopic Treatment of a Spontaneously Ruptured Kidney (Wunderlich Syndrome)
Spontaneous, nontraumatic retroperitoneal hemorrhage or Wunderlich syndrome (WS) is a rare but potential life-threatening condition. In most patients a bleeding renal neoplasm is the cause of the retroperitoneal hematoma. The management of this condition includes a conservative approach in the hemodynamically stable patients and active treatment in the unstable patients. Active treatment includ...
متن کاملLaparoscopic Retroperitoneal Nephron-Sparing Surgery Without Renal Artery Clamping with Preoperative Selective Arterial Embolization for Management of Right Renal Angiomyolipoma of Diameter 10 cm: A Case Report
A 38-year-old female without the tuberous sclerosis complex was diagnosed with right renal angiomyolipoma of 10 cm in diameter. She underwent laparoscopic retroperitoneal nephron-sparing surgery without renal artery clamping with preoperative selective arterial embolization to avoid a significant risk of hemorrhage and the damage of the renal function during nephron-sparing surgery. The tumor w...

متن کاملNephron Sparing Surgery for Renal Angiomyolipoma with Inferior Vena Cava Thrombus in Tuberous Sclerosis
Introduction. Angiomyolipoma is a common benign renal tumor. It is associated with Tuberous Sclerosis Complex (TSC) in 20% of patients. Angiomyolipomas are classically multiple, bilateral, and growing; they may lead to complications such as Wunderlich syndrome or, in rare cases, to venous extension. Observation. a 74-year-old woman with TSC presented with an angiomyolipoma of the right kidney w...
متن کاملElective nephron sparing surgery for renal cell carcinoma larger than 4 cm.
PURPOSE Elective nephron sparing surgery is established as an alternative to radical nephrectomy for renal cell carcinoma if tumors are small (4 cm or less, stage T1a). We compared outcomes in patients with renal cell carcinoma 4 cm or less (small) vs more than 4 cm (large) who were treated with nephron sparing surgery. MATERIALS AND METHODS Between 1979 and 2006, 618 patients underwent elect...
